Occupational Therapist Job Description:

Job Summary:

Responsible for the evaluation, diagnosis, prognosis, development, implementation and documentation of treatment plans for patients/clients to relieve pain, restore function, prevent/reduce disability, and to improve cognitive and/or perceptual limitations, ranging from pediatrics to geriatrics with the goal of maximizing functional independence.  Delivers this service according to the standards of care defined within the State of Maine Occupational Therapy Practice Act.

Essential Job Duties:

  1. Provide accurate and thorough initial evaluation and re-assessments for patients in a variety of settings. Varieties to include but not limited to:
  2. Inpatient, outpatient, homecare, skilled nursing, school, nursing facilities, and contracted services.
  3. Develop appropriate individualized treatment plans aligning goals of patient, family, payor, and caregiver.
  4. Deliver highest quality of patient care.
  5. Document daily services rendered, including intiail evaluation, payor certification forms, daily progress notes, charges, and discharge summaries.
  6. Documentation within 72 hrs. for hospital outpatient service.
  7. Payor certification form within 24 hrs. of service.
  8. Inpatient hospital documentation within 24 hrs. of service.
  9. Inpatient hospital documentation within 24 hrs. of service.
  10. Documentation for contracted services shall be according to contract standards.
  11. Communicate with provider to inform him/her of patient’s initial status, progress, and discharge status through written documentation and verbal communication as necessary.
  12. Confer with Primary Charge of Rehabilitation Services problem issues and ideas for resolution.
  13. Demonstrate active role in program development and marketing.
  14. Participates in research or program development.
  15. Initiates and/or participates in implementation of programs.
  16. Initiates and/or participates actively in education of hospital staff of new programs.
  17. Initiates and/or participates actively in marketing of new programs.
  18. Demonstrate current, quality work as enhanced by professional journals, workshop attendance, and organization participation.
  19. Perform duties in a productive manner.
  20. Foster communication and demonstrate positive, respectful relationships with rehabilitation co-workers, personnel from other hospital departments, personnel from contracted services, and other customers.
  21. Participate in education activities:
  22. Cary Medical Center’s Health Fairs.
  23. Staff lectures.
  24. Other hospital related events.
  25. Assist in quality management:
  26. Determine indicators
  27. Collect and analyze data
  28. Determine problem areas
  29. Implement solutions
  30. Demonstrate outstanding customer service skills.
  31. Perform other duties as assigned.

Other Responsibilities:

  1. Complies with local, state, and federal laws and regulations.

License Requirements:

CPR Certified and Basic First Aid Certification eligible. Valid driver’s license. NBCOT Registration and Maine State Licensed.

Availability Requirements:

Must be available to work Monday through Friday, with occasional weekends and holidays. Hours depend on customer and department needs, flexibility required. Able to drive amongst Cary’s work locations and contract sites.

 

Certified Occupational Therapist Assistant Job Description:

Job Summary:
This individual is responsible for assisting the occupational therapist in the delivery of occupational therapy services by implementing treatment plans based upon a licensed therapist’s evaluations. Caseload may include any age inpatient, outpatient, and/or contracted services client.

Essential Job Duties:

  1. Create and update individualized treatment plans for inpatients, outpatients and contracted services patients along the age spectrum based upon the occupational therapist’s evaluations and recommendations.
  2. Provides treatment outlined in the therapist’s plan. Treatment may include but not be limited to therapeutic exercises and activities, application of modalities, community re-entry programs, and homecare.
  3. Implement treatment plans under the indirect supervision of an occupational therapist.
  4. Participates in planning for patient discharge and equipment ordering.
  5. Instructs patients, families, and/or primary care givers in therapeutic interventions to meet the needs of the patient.
  6. Document accurately daily services rendered, including daily notes, insurance forms, and charge sheets.
  7. Foster communication and demonstrate positive relationships with team members and with staff in other departments.
  8. Communicate with medical providers through written documentation, and when appropriate, verbal communication.
  9. Assist with program development and marketing by discussing ideas for developing programs or problem resolution with the Primary Charge of Rehabilitation Services.
  10. Follows departmental and Cary Medical Center (CMC) policies and procedures.
  11. Responsible for maintaining current knowledge in the field through professional journals, workshops, and participation in professional organizations.
  12. Participate in community educational activities or in-service programs as a presenter or attendee. This includes participation in CMC’s Health Fairs, community lectures, and other hospital related events.
  13. Assist with determining indicators, collecting and analyzing data, problem solving, and implementing solutions for Quality Management as requested by the Primary Charge of Rehabilitation Services.
  14. Demonstrate strong customer service skills.
  15. Performs other work related duties as assigned.

Other Responsibilities:

  1. Complies with local, state, and federal laws and regulations.

License Requirements:

CPR and basic First Aid eligible. Registered COTA with Maine License. Valid driver’s license or actively pursuing one.

Availability Requirements:

Must be available to work Monday through Saturday, with occasional Sundays and holidays. Hours depend on customer and department needs, flexibility will be needed. Able to travel amongst Cary’s locations and contract sites.
